How do I actually get and use it?

Do I need a Claude account already?
Yes. This runs inside Claude, not on COSH!'s servers. It works on Free, Pro, Max, Team, and Enterprise plans; you just need "Code execution and file creation" turned on in your Claude settings. If you've never used Claude, a free account is enough to install and run it.
